If you are paying by credit card, please submit this form and phone Lily within 2 weeks in the Or Shalom Office to provide your credit card information.

If you are paying by cheque, please drop off your cheque at the office within 2 weeks. Please write "Retreat" and the name of the first participant on your cheque so we can match it up to your online registration.
